Entry Health Declaration System (Aircraft version)
依傳染病防治法規定，旅客應詳實申報本系統資料。填寫不實者，依該法第69條規定，最高得處新臺幣15萬元以下罰鍰。
According to Article 69 of the Communicable Disease Control Act, any person who falsifies on this notice shall be fined up to NT$150,000.

  1. 所有入境航班旅客過去14天內如有發燒、咳嗽、流鼻水鼻塞、呼吸急促、腹瀉、嗅味覺異常、全身倦怠及四肢無力等症狀，請於抵臺時主動洽CDC檢疫人員，配合健康評估及完成本系統健康申報。
  2. 本人同意「入境旅客配合採檢措施及健康管理敬告單」或「有症狀旅客健康評估暨檢疫敬告單及提審權利告知」，以電子表單方式收執。 持大陸地區門號或漫遊門號者，為順利接收簡訊憑證，建議請先向電信業者開通相關服務。
  1. Inbound passengers who have experienced fever, cough, runny/ stuffy nose, shortness of breath, diarrhea, loss (or change to) sense of smell or taste, fatigue, or limb weakness in the past 14 days should voluntarily inform Taiwan CDC quarantine officers; they should also cooperate with quarantine officers in undergoing a health assessment and completing the health declaration form in this system.
  2. I agree to receive and keep a copy of the “Notification of Compulsory Specimen Collection and Health Measures for Inbound Passengers” or the “Notification of Compulsory Quarantine for Arriving Passengers with Suspicious Symptoms and Right to Petition for Habeas Corpus Relief” in electronic form. To receive an SMS in Taiwan, please apply to activate relevant international services with your telecom service provider if you have a Chinese mobile phone number or international roaming phone number.
